Our office uses an electronic medical record system by Athenahealth which allows state of the art documentation and medical record keeping, while extending our services online and providing added convenience to the patients.
1. Before you arrive
Your referring physician provides the initial new patient request.
You will then need to fill out the New Patient Registration Form listed below and mail it to our office. As soon as we receive this form, we will schedule your appointment and you will have access to our patient portal.
Completing the online check-in using the patient portal before you arrive will ensure quick service and shorter waiting times.
Please bring your insurance card and a valid photo ID for your appointment.

3. After your appointment
You will receive a patient summary of care, along with an action plan to treat your conditions. This information will also be available to download through your Patient Portal log in. We will send all prescriptions electronically directly to your pharmacy of choice. We will schedule all your tests and contact your insurance provider to obtain their approval before we conduct any tests.
